Disease Management Information Toolkit

Disease management information toolkit (DMIT) is a good practice toolkit designed to help increase the efficiency of services for children with long-term conditions. It was originally developed by the Department of Health for the NHS to help service providers identify which conditions contribute to high numbers of emergency bed days.

This toolkit is primarily aimed at primary care trusts (PCTs) and commissioners of children's health and social care services, although it will inform those working with people with long-term conditions at all levels.

The toolkit uses data already in the public domain to present information in a user-friendly format. It also provides a summary report of key data and information on trends over time and potential cost savings.

Paediatric modules

Modules for paediatric asthma, diabetes and epilepsy are available.

The Disease Management Information Toolkit (DMIT) has information at Primary Care Trust (PCT) level on emergency hospital admissions for patients aged under 19 with a primary diagnosis of a particular condition. The toolkit enables PCTs to compare their emergency admission rates, bed-days and lengths of stay with a range of different comparators. It is designed to highlight variations at PCT level and allow benchmarking to inform the commissioning decision-making process for children's services.

Use the DMIT Paediatric modules

Sponsored by the Department of Health, ChiMat has updated and relaunched the first module in the DMIT toolkit – Paediatric asthma. Modules for diabetes and epilepsy are also now available.

The toolkit uses data from the Information Centre for Health & Social Care Hospital Episodes Statistics (HES) data warehouse on emergency admissions, emergency bed days and length of stay.
